MI Hospice Residence Licensure Process

Requirements
Applicants are required to have been licensed as a hospice program pursuant to Public Act 368 of 1978, as amended, and be in compliance with the conditions and standards set forth in the Conditions of Participation for Medicare Hospice Certification  (42 CFR, Part 418) for not less than two years immediately preceding the date of application for residence licensure. Additionally, the proposed hospice residence must be in compliance with the Construction/Design Requirements and Life Safety Codes as amended.

The process for becoming a hospice residence provider is as follows:

1.

Contact the following offices to obtain the necessary Life Safety Code and Construction/Design approvals prior to being issued a construction permit and to submit the Application for a Hospice Residence License form (BHS-LC-107c) to the Division of Licensing & Certification.

The physical plant will be inspected for compliance by the Bureau of Fire Services and the Health Facilities Engineering section.  Final approvals for occupancy of the residence will be issued to the agency and copies of the approval sent to the Division of Licensing & Certification.

The applicant is to contact the Division of Licensing & Certification for an onsite to ensure the agency is prepared to begin patient care. The Division will assign a hospice surveyor to conduct an onsite visit to review the agency's readiness to begin patient admissions. Patients may not be admitted to the hospice residence until a license is issued.

The agency will be invoiced a residence license fee of $200 plus $20 per each approved residence bed once all required documents and onsite visit are completed.

A hospice residence license will be issued to the agency upon department approval. This is usually done in coordination with the agency's expected date of first patient admission. 

6.

The licensed residence hospice may then admit patients. The agency is to contact the Licensing & Certification Division to indicate readiness for a State survey when a caseload of 3 to 5 patients is reached. 

7.

An unannounced survey of the hospice residence will be conducted to evaluate compliance with all licensure requirements.

Relicensure
Annual fees ($200 + $20/bed) are based on a billing cycle of August 1st to July 31st. Please provide updated facility information by completing the Facility Information Sheet for Licensed Facilities form (BHS-LC-101) and sending it to the Division of Licensing & Certification by either faxing, e-mailing the attachment to dch-bhs-l&c@michigan.gov or by mailing it to the address shown above.
