Michigan Hospice Program Licensure

Requirements
Applicants must be licensed with the State of Michigan before Medicare certification is approved. However, agencies may submit the necessary forms to begin the process for Medicare certification at the same time as applying for a license. (See the Hospice Medicare Provider Licensure Process page.) The process for becoming a licensed hospice provider is as follows:

Review State of Michigan Rules for Hospices and Hospice Residences (R 325.13101-325.13418) and begin to build the hospice program components according to those State requirements. The Division of Licensing & Certification staff will help clarify key steps and typical time lines for opening a hospice agency upon request (see Contact Information below).

Hospice license applications are now managed through Michigan Business One Stop. If you already have a One Stop account, go to www.michigan.gov/business to login. If you are new to One Stop, see How do I use Michigan Business One Stop? for instructions on setting up an account, adding your business to your account, and applying for a hospice license. Contact the Customer Assistance Center at 877-766-1779 if you have questions about One Stop.

Contact the Division of Licensing & Certification by phone or letter when in compliance with State rules (#1) and ready to begin patient admissions.

The Division will assign a hospice surveyor to conduct an onsite announced visit to review the agency's readiness to begin patient admissions. Documents such as the patient admission materials, patient rights' information, consent forms, policies and procedures, job descriptions, staffing agreements, contracts for services including for inpatient care, and volunteer and bereavement program policies will be reviewed during the evaluation visit.

A license will be released to the agency upon Department approval.

The licensed hospice may then admit patients for home care services. The agency should contact the Division of Licensing & Certification to indicate readiness for a State survey when a caseload of 3 to 5 patients is reached.

7.

An unannounced hospice licensure survey to evaluate compliance with all licensure requirements will be conducted.

There is no fee for hospice licensure at this time. New licenses are issued automatically each year.

Applicants interested in operating a Michigan licensed hospice residence are required to have been a Michigan licensed hospice program for not less than two years immediately preceding the date of application for licensure and meet other requirements.
