Quality Improvement Nurse Consultation for Long Term Care (QINC) Program

LONG TERM CARE NOW HAS...

 

Quality Improvement Nurse Consultants! This program has been developed within the Bureau of Health Systems to work collaboratively with nursing homes throughout the state. We will provide you with focused, proactive and preventative information to improve resident outcomes and delivery of care by providers.

 

Based on Quality Indicators triggered by the Minimum Data Set, Quality of Care trends identified in the Long Term Care industry, and requests generated by the facilities themselves, the Nurse Consultants will make mutually desirable visits. Our evaluative framework will focus on structure, process, outcome and quality assurance tools that are consistent with your facility needs.

 

Together we will identify problems within a system that impede the resident's highest level of functioning. Best standards of clinical practice will be shared, recognizing that variances may exist based on each facilities' uniqueness.
