Referring Your Patients to MCB

• MCB Office Locations & Staff Directory:
The Michigan Commission for the Blind (MCB) Central Administrative Office and the MCB Braille and Talking Book Library are both in Lansing. The residential MCB Training Center is in Kalamazoo. MCB Regional Offices are located in Detroit, Escanaba, Flint, Gaylord, Grand Rapids, Kalamazoo, Lansing, and Saginaw. Office contact information and a complete staff directory (alphabetical by name and alphabetical by name at each location) are included here. (Updated 2-14-12.)

• MCB Eye Report Form PDF icon
Use this form to refer eligible blind and visually impaired patients age 14 and over to the Michigan Commission for the Blind. This form may be filled out electronically and then printed for doctor and patient signatures, or the form may be printed blank and filled out and signed by hand. Please fax or mail the completed form to the MCB central office in Lansing or your local MCB office. This form was updated April 24, 2011, with the new name of the department that includes the Michigan Commission for the Blind: the Michigan Department of Licensing and Regulatory Affairs (LARA).
