Relicensure

Licensees who miss the late renewal period and allow their license to lapse may apply for relicensure.

Requirements for licensees lapsed from 61 days to 3 years include the filing of the appropriate application and fees, and that the applicant be of good moral character.

Requirements for a licensee lapsed for more than 3 years include the filing of the appropriate application and fees, that the applicant be of good moral character, and that the applicant meets current requirements for education, experience and examination.

Applications for relicensure may be requested by e-mail, telephone, or in writing.
