  • BSBP Eye Report Form PDF iconUse this form to refer eligible blind and visually impaired patients age 14 and over to the Bureau of Services for Blind Persons. This form may be filled out electronically and then printed for doctor and patient signatures, or the form may be printed blank and filled out and signed by hand. Please fax or mail the completed form to the Bureau central office in Lansing or your local Bureau office. This form was updated October 1, 2012, with the name of the new bureau (the Bureau of Services for Blind Persons) created by Governor Snyder's Executive Order 2012-10.
