Attachment C of the OmniCare First Amendment to the Rehabilitation Plan contains a detailed schedule of medical claim liabilities owed to provider creditors.

Creditors may inquire on specific medical claim liabilities by performing the following steps:

1. Open Attachment C - Schedule of Medical Claims Liabilities (618 KB)
2. To find claims for a specific vendor, you must know the vendor ID number. You can use the Adobe Acrobat Find command to locate a vendor ID. Here is how it works:

• Click the Find icon on the Adobe toolbar.



• Enter the vendor ID in the text box:




• Select the search options if necessary:
  1. Match Whole Word Only finds only occurrences of the complete vendor ID you enter in the text box. For example, if you search for vendor ID 123, the vendor ID 56123 and 12345 will not be highlighted.
  2. Match Case finds only vendor ID's that contain exactly the same capitalization you enter in the text box.
  3. Find Backwards starts the search from the current page and goes backward through the document.
     
     
• Click Find. Acrobat finds the next occurrence of the vendor ID.



• To find the next occurrence of a vendor ID, click the Find Again icon on the Adobe toolbar.


Netscape Users: The Find Again feature is available only on versions 4.08 and higher.
