Division of Licensing and Certification

Bureau of Health Systems(Home)

The Division of Licensing and Certification is within the Bureau of Health Systems (BHS), Michigan Department of Licensing and Regulatory Affairs (LARA).

The Division is responsible for state licensing and federal certification of the following health facilities and agencies as well as providers and suppliers.

State Licensing

The Michigan Public Health Code requires the following health facilities and agencies to be state licensed (Codes and Rules):

Clinical Laboratories
Hospice Agencies and Residences
Hospitals
Freestanding Surgical Outpatient Facilities (FSOF)
Psychiatric Hospitals/Units and Partial Hospitalization Programs

Federal Certification

The Bureau of Health System is the State Agency (SA) for Michigan providers that want to be federally certified to participate in the Medicare/Medicaid programs. The Division coordinates the certification program on behalf of the Centers for Medicare and Medicaid Services (CMS) for the following provider and supplier types. The certification process and priorities are set by CMS.

Ambulatory Surgical Centers (ASC)
Clinical Laboratories
Comprehensive Outpatient Rehabilitation Facilities (CORF)
End Stage Renal Dialysis (ESRD) Facilities
Home Health Agencies
Hospice Agencies and Residences
Hospitals
Outpatient Physical Therapy (OPT) Facilities
Portable X-Ray Suppliers
Psychiatric Hospitals/Units and Partial Hospitalization Programs
Rural Health Clinics

Please note: Facilities not covered by the Division of Licensing and Certification include assisted living, state operated facilities (unless federally certified), adult foster care, and homes for the aged.

Complaints

A complaint or allegation against a state licensed or federally certified provider may be made verbally, in writing, or anonymously by calling the Bureau of Health System's Complaint Hotline at 1 800-882-6006, by Faxing to (517) 241-0093, or filing online. The Health Facility Complaints and Investigations web page will provide more information.

Please note that complaint investigations are focused on if a provider has violated state licensing requirements or a condition of participation in the Medicare/Medicaid program. Other available complaint hotlines below:
