Home Health Agencies

Back to Division of Licensing and Certification

A "Home Health Agency" is an organization primarily engaged in providing skilled nursing services and other therapeutic services to patients in their own homes.

State Licensing

The State of Michigan does not require state licensing for home health agencies.

Federal Certification

The Division of Licensing and Certification is the State Agency on behalf of the Centers for Medicare and Medicaid (CMS) for home health agencies in Michigan. The Division makes recommendations to CMS on a variety of issues including, but not limited to, whether a potential home health agency meets the Conditions of Participation for certification in the Medicare program, conducting complaint investigations, and recertifying existing providers. State agencies do not have Medicare determination-making functions or authorities.

CMS Enrollment provides an overview on how providers can apply for enrollment in the Medicare program or make a change in their existing enrollment information (see Tips to Facilitate the Medicare Enrollment Process). To enroll or change provider information (Change of Ownership, location, etc.), complete a CMS 855 form and submit to the applicable Fiscal Intermediary (FI)/MAC (see Provider Enrollment Contact List in Downloads on CMS Enrolment page). Please note that submission to the FI/MAC may include an application fee. This first step is important to start the federal certification process or keep existing information current.

Additional state and federal forms to be submitted to the State of Michigan, as the State Agency, includes Health Insurance Benefits Agreement (CMS 1561), Home Health Agency Survey and Deficiencies Report (CMS 1572A), Civil Rights, and Michigan OASIS System Agency Application (BHS-LC-150).

CMS Accrediting Organizations. CMS permits providers and suppliers "accredited" by an approved national accreditation organization (AO) to be exempt from routine surveys by State survey agencies to determine compliance with Medicare conditions (see Downloads for Approved AO by provider/supplier type). Accreditation by an AO is voluntary and is not required for Medicare certification.

Complaints

A complaint or allegation against a federally certified provider may be made verbally, in writing, or anonymously by calling the Bureau of Health System's Complaint Hotline at 1-800-882-6006, by faxing to (517) 241-0093, or filing online.

Please note that complaint investigations are focused on if a certified provider has violated a Condition of Participation in the Medicare program. For other complaint hotlines, see Division of Licensing and Certification home page.
