The Facility Incident Report (BHS‑OPS-362) form has been modified for reporting alleged/possible resident mistreatment, neglect, abuse or injury of unknown source, or the misappropriation of property in nursing homes.
A Facility Incident Investigation Report form (BHS OPS-363) has been added to report the facility's investigation findings.
The Complaint Investigation Manual is in the process of being updated to reflect these changes.
This will avoid duplication of data entry in the database and ensure that the full notification process has been followed.
Clarification of the required notification:
1. Required telephone notification to the Complaint Hotline (1-800-882-6006). State and federal laws require "immediate" reporting. "Immediate" means as soon as possible, but not more than 24 hours after the discovery of the incident.
2. Completion and submission of the Facility Incident Report (BHS OPS-362) within 24 hours of the incident, and
3. Completion and submission of the Facility Incident Investigation Report (BHS OPS-363) form reporting the facility investigation findings within 5 working (state) days of the incident.
