Request a MAPS Report

Upon registration to MAPS Online and all requests for MAPS information, all users are required to review, acknowledge and certify that the information they are requesting shall be used only for the purpose of providing medical or pharmaceutical treatment to a bona fide current patient and that they will not provide this information to any other person or entity except for other practitioners who are treating the same patient. HIPAA and all confidentiality and disclosure provisions of Michigan Law cover the information obtained in the MAPS database. These requirements include, but are not limited to:

It is encouraged that practitioners and law enforcement obtain MAPS reports through MAPS Online. Reports requested through MAPS Online are available almost immediately upon request.

Faxed requests using the forms below can take, on average, 48 hours to process.
