MAPS Information for Veterinarians

All controlled substances dispensed by a practitioner, including veterinarians, are required to be reported electronically through MAPS Online. Practitioners must register to MAPS Online to submit prescription data and to request controlled substance history reports on their patient(s):

When entering prescription data into MAPS for controlled substances dispensed for animals, please keep in mind the following tips:

* Customer ID -- Use the pet owner's identification (driver's license or Michigan I.D. card) for all controlled substances dispensed for animals.

* Birth Date -- Use the pet owner's date of birth.

* Patient First Name -- Use the pet owner's first name.

* Patient Last Name -- Use the pet owner's last name.

* NDC Code -- Entry requires 11 digits.

* Rx Number -- The system will not accept letters or dashes (-,/); enter only a maximum of 7 digits.
