Updated Incident Reporting FormsAgency: Licensing and Regulatory Affairs

It has become apparent that incidents are not consistently reported. New incident reporting forms have been developed to improve consistency using clear definitions and instructions from Michigan statutes and the newly adopted ASTM Standards. The development of these forms has included representatives of the State, industry and public and the forms have been endorsed by the Carnival-Amusement Safety Board.
The new forms can be downloaded from "Forms & Publications" under "Quick Links" and the changes, differences, definitions and rationale are described on the pages listed below.
