Resident Trainee

The Resident Trainee license expires on January 31st of every year.  The renewal form will be sent to the sponsor Funeral Establishment that is on file with the Department approximately 60 days prior to the date of renewal. The license may be renewed online with a credit card or by mail with a check or money order.  The renewal fee is $30.

Mortuary Science License

The Mortuary Science License expires on October 31st of every odd-numbered year.  The renewal form will be sent to the home address that is on file with the Department approximately 60 days prior to the date of renewal.  The license may be renewed online with a credit card or by mail with a check or money order.  The renewal fee is $80.

Funeral Establishment

The Funeral Establishment license expires on October 31st of every odd-numbered year.  The renewal application will be sent to the Funeral Establishment address that is on file with the Department approximately 60 days prior to the license expiration date.  The renewal can be completed online with a credit card or by mail with a check or money order.  The Funeral Establishment manager's license must be renewed before the Funeral Establishment license will be issued.  The renewal fee is $110.

Change of Address

Licensees must ensure the Department has the most current address on file so that the renewal application will be received.
