Application packets are available for downloading at each profession's page by clicking on the Health Professions button on the left and then selecting your profession from the drop down menu. To request an application packet to be mailed to you email us at firstname.lastname@example.org or call 517-335-0918. To check the status of your Michigan license application online, go to www.michigan.gov/appstatus or call 517-335-0918.
Board of Medicine applicants applying for a full license, or Board of Medicine and Board of Osteopathic Medicine applicants applying for either a clinical academic or educational limited license may apply online at www.michigan.gov/elicense.
To change your name you must submit the request in writing. To change your address you must either submit it in writing or via the Internet. A new license is not issued automatically. You can complete the Data Change/Duplicate License Request Form and fax it to 517-373-2179 (no new license issued) or mail the form to the Health Professions Licensing Division, Bureau of Health Care Services, PO Box 30670, Lansing MI 48909. Include the $10 fee if a new license is required. To change your address online go to www.michigan.gov/elicense. If you require a new license to be issued with the new information you can request and pay for a duplicate license with a credit card.

Michigan's nursing licensure program utilizes NURSYS® electronic license verification system. Michigan licensed RNs and LPNs applying for licensure in another state should contact Nursys at www.nursys.com to have their Michigan license verified. A fee will be charged for that verification. If you have questions about Nursys please contact them toll free at 866-819-1700. If you hold a Michigan RN license and a specialty license you must obtain your verification from the Michigan Board of Nursing. See below for information on how to obtain written verification.
Michigan licensed MDs and DOs must contact VeriDoc at www.veridoc.org to have their license verified to another state. A fee will be charged for that verification. Questions should be directed to VeriDoc at 701-319-6500.
For all other professions to obtain written verification of your license to another state submit request in writing to the Verification Unit, Health Professions Licensing Division, Bureau of Health Care Services, PO Box 30670, Lansing MI 48909 along with $15 fee (check or money order payable to State of Michigan) for certification processing. Include name as it appears on your license, license number and name/address of state board the certification should be sent. Normal processing time is 20-30 business days.
