How can I get disciplinary information about health care professionals?

Disciplinary records regarding licensed or registered health care providers are public information pursuant to the Freedom of Information Act. Use any method described below.
  • Current Disciplinary Action Reports 
  • Past Disciplinary Action Reports: Electronic copies of reports from 2008-2013 are available at http://worldcat.org/oclc/70910395/viewonline). Print copies of reports from 1993-2013 are available for review at the Library of Michigan at www.michigan.gov/mde/0,1607,7-140-54504---,00.html.  
  • Telephone Verification: 517-241-7849
  • Written Request: Department of Licensing and Regulatory Affairs, Bureau of Health Care Services, Administrative Support Division, Freedom of Information Coordinator, PO Box 30670, Lansing, MI 48909. Include your name and address; the name of the health care professional; the license or registration type; and, a detailed description of the information being requested.
