Freedom of Information Act (FOIA) Requests

Bureau of Community and Health Systems - Requesting Documents

Requests for documents under the Freedom of Information Act (FOIA) must be submitted in writing to the Bureau of Community and Health Systems. Requests must include a sufficient description of the records you are seeking and your contact information in order for a response and records to be sent to you.

Under FOIA, our office has five (5) business days to respond to a request, but may extend the time by an additional 10 business days by notifying the requesting person in writing of the reason for the extension and the date a response shall be provided.

In addition, a fee may be charged for the search, retrieval, examination, review, and separation and deletion of exempt from nonexempt material.  If a fee is charged, an invoice will be sent for any applicable costs for the request.
