Freedom of Information (FOIA)

Freedom of Information Act (FOIA) requests regarding occupational and health professions must be submitted in writing directly to the Legal Affairs Division in the Bureau of Professional Licensing. Requests must include a description of the records you are seeking and your contact information in order for a response to be sent to you. Please allow five business days for us to respond to your request.
