This page provides information regarding the license look up web site as well as other ways to obtain information you are seeking regarding a license or registration.

The Bureau of Health Services License Lookup web site gives the following information on licensees/registrants:
  • Current license status (active, inactive or pending)
  • License Number
  • License issue date and expiration date
  • An indication as to whether an open formal complaint currently exists
  • Disciplinary action information
