MCOLES Network Forms & Addendums

Agency User Agreement Addendums should be used to make changes to your agency's original User Agreement previously on file with MCOLES. All signed, completed forms should be FAXED to the Licensing Services Section @ 517-316-0824.  Signed documents can also be scanned and e-mailed as a PDF attachment to Rhonda Hooson @ hoosonr@michigan.gov.

Why should I use this document? 
Law Enforcement Agency User Agreement Addendum   *  

Law enforcement departments should use this document to change the name of the agency head, change the Network Point of Contact, and retain or rescind individual Operator rights.

Addendum for a Law Enforcement Agency to Add Criminal Justice In-Service Training Provider Rights    *    

Law enforcement departments should use this document if they want to become an in-service criminal justice training provider and enter training courses into the Network.

Criminal Justice In-Service Training Agency User Agreement Addendum    *    

Current In-service training agencies should use this form to change the name of their agency head, change the Network Point of Contact, and retain or rescind current Operator rights.

Basic Training Academy Agency User Agreement Addendum   *   

Michigan basic training academies should use this document to change their agency head, change the Network Point of Contact, and retain or rescind current Operator rights.

Law Enforcement Officer Access Agreement   *    

Effective October 1, 2007, individual law enforcement officers can request access to the MCOLES Network to view their profile information.  This form starts the process to obtain a secure User ID and password to log into the Network.

Network Operator Agreement  * 

Individuals new to the Network should use this form to apply as an Operator on the Network. Attendance at a training session is required for all law enforcement agency operators. 

Network User Agency Agreement *  

New agencies should submit this form to become an authorized user on the Network.

License Activation & Record Retention Check List  Agencies should use this form as a check list when hiring a new individual as a law enforcement officer.
