This information complements the contact list of persons requiring notification due to medical conditions as required under Regulation 637, Rule 5.  The contact list and the access code for the address list was mailed to licensed firms that apply pesticides in the following categories:  3A, 3B, 6, 7F, Aerial.

• Pesticide Notification Quick Reference - This is a quick guide to assist you in determining if you have customer addresses that appear on the 2009 registry (45 pages total).   The guide provides you the page numbers of the registry to print if you do not want to print the entire registry, which is found at www.michigan.gov/mda-reg637registry.   It is your responsibility to print the correct pages if you do not print the entire registry.   The access code needed to open the document was mailed to licensed firms.   Contact Marie White at 517-241-1169 for more information.
