Below please find links to the frequently use forms and documents for the Youth Horse Racing (YHR) Program.
Many of these are in Adobe PDF format, and will require Adobe's free Acrobat Reader to view.

Rules and Regulations:	A complete list of the rules and guidelines that must be followed at each YHR event.
Fair Information Form:	This form must be completed by the hosting fair. It provides Michigan Department of Agriculture and Michigan Thoroughbred Owners and Breeders Association staff with the needed contact and program information to effectively prepare for and complete the program.
Participant Information Form and Liability Waiver:	This must be completed by all youth participants and their guardians prior to their participation in *each and every* YHR event. The form provides participant & horse information, emergency contact information, and a medical release form. This form must be completed for each event the youth participates in, regardless.
Certification Test:	This is an example of the test each participant, with their horse, must complete in order to be certified to compete in the race. A participant and his or her horse must only be certified once each year. Once certified, the team will be provided a certification card to present at future races throughout that year.
Participant Safety Checklist:	A checklist each participant and horse team must go through just prior to the race to verify the jockey equipment is safe and being used correctly.
Heat Breakdown:	A form used by host coordinators to divide participants into heats for racing.
Race Tack Diagram:	The diagram provides an idea of the race procedures and track layout used for the YHR program
YHR Overview Handout:	A summary of the Youth Horse Racing program used as a handout for interested individuals.
YHR PowerPoint:	A power point presentation used to highlight the YHR program to interested groups and individuals. Download the PowerPoint in PDF format.
