To use the MRAP Reimbursement Form:

1. To open the file, click on the appropriate link.
2. Select "File," "Save As" from your browser menu.
3. Save the document to your PC. Be sure to remember which folder you save the file to.
4. Open the form and fill it out.
5. When completed, save and close the document.
6. Email your completed MRAP Reimbursement Form to the appropriate Michigan Works! Agency Representative.
