Community Assessment and Awards
This website, resources, and tools were developed by the Michigan Department of Community Health (MDCH) and several partners to address the health crisis that citizens are experiencing, such as obesity or being at risk for heart disease or stroke. The assessments are meant to assist community coalitions in assessing their policies and environment and to help gain knowledge on how they can make their community more supportive of being physically active, eating healthier, and living a tobacco free life. Tools included are Healthy Community Checklist, Promoting Active Communities, Nutrition Environment Assessment Tool, and Smoke-Free Community Assessment Tool. The assessment tools can all be completed free online, and automatic reports will be generated. Resources and technical assistance are also provided through MDCH.

Healthy Communities Tool Kit
The Healthy Communities Tool Kit was created in an effort to be true to our vision and mission, as well as to guide healthy community coalitions though the process of creating policy and environmental change. The tool kit includes sections that specifically focus on opportunities for physical activity, access to fruits and vegetables, and tobacco-free environments.(2006)
