Application packets are available for downloading within each profession's link listed on the left. If you'd like an application packet mailed to you, e-mail us at bhphelp@michigan.gov or call (517) 335-0918. Board of Medicine applicants can apply online at mylicense.
To change your name you must submit the request in writing. To change your address you must either submit in writing or via the Internet. A new license is not issued automatically. You can complete the Data Change/Duplicate License Request Form and fax to (517) 373-2179 (no new license issued) or mail the form to MDCH/BHP, PO Box 30670, Lansing, MI 48909. Include the $10.00 fee if a new license is required. To change your address online go to www.michigan.gov/mylicense. If you require a new license to be issued with the new information you will be able to request and pay for a duplicate license with a credit card.

Effective January 1, 2009 Michigan licensed MDs and DOs should contact VeriDoc at www.veridoc.org to have their license verified to another state. A fee will be charged for that verification. Questions should be directed to VeriDoc at (701) 319-6500.
For all other professions to obtain written verification of your license to another state submit request in writing to MDCH/BHP, Verifications, PO Box 30670, Lansing, MI, 48909 along with the $15.00 fee (check or money order payable to State of Michigan) for certification processing. Include name as it appears on your license, license number and the name and address of the state board to which the certification should be sent. Normal processing time is 20-30 business days.
