| How can I get disciplinary information about health care professionals? |

Disciplinary records regarding licensed or registered health care providers are public information, pursuant to the Freedom of Information Act. Use any method described below.
- Verify a license
- Current Disciplinary Action Reports
- Local Public Libraries: Ask your local public library for a copy of the annual Disciplinary Action Report of the Bureau of Health Professions, Michigan Department of Community Health.
- Telephone Verification: (517) 241-9427
- Written Request: Department of Community Health, Bureau of Health Professions, Freedom of Information Coordinator, PO Box 30670, Lansing, MI 48909-8170. Include your name and address; the name of the health care professional; the license or registration type; and, a detailed description of the information being requested.
