Nursing Home Informal Deficiency Review (IDR) Process

The Bureau of Health Systems has established the Informal Deficiency Review (IDR) for Long Term Care Facilities process for the purpose of resolving disputes over deficiencies cited by survey staff. Facilities having disputes over citations not resolved at the time of the survey or immediately after may request a third-party review known as the Informal Deficiency Review (IDR), a process explained in this link.

The facility must complete the Informal Deficiency Review form (BHS-108) for each disputed deficiency to utilize this process.  BHS-108e is the fill-in version of this form: Informal Deficiency Review.
