Initial Application Process

●   New Psychiatric Inpatient Programs

      Interested applicants must first secure a Certificate of Need (CON) approval to initiate a Psychiatric Inpatient Program. The link, Filing a CON Application, provides all necessary information needed to understand the initial application review.  Applicants that have received CON approval should complete the Bureau of Health Systems' Psychiatric Program License Application (BHS-LC-820), attachments (shown in the Application and Fees  section below) and the appropriate license fee at least 30 days prior to the projected start date of the approved new psychiatric program. 

●    Psychiatric Partial Hospitalization Programs

      Applicants interested in starting a Psychiatric Partial Hospitalization Program should submit the Bureau of Health Systems' Psychiatric Program License Application (BHS‑LC‑820), attachments (shown in the Application and Fees section below) and the appropriate license fee at least 90 days prior to the projected start date of the approved new psychiatric program. 

License Renewal

Psychiatric Inpatient Unit and Psychiatric Partial Hospitalization Program licenses must be renewed every two years based on the initial license date. The Psychiatric Program License Application (BHS-LC-820) and attachments (shown in the Application and Fees section below) may be downloaded from this website, completed and mailed along with the renewal fee to the Bureau of Health Systems approximately two months before the license expiration date, to allow time for processing. 

Application and Fees

An applicant for a license or renewal of a license shall submit to the Department with the application form a license fee of $600.00 plus $7.50 per patient bed or treatment position. Checks should be made payable to the State of Michigan.
