The Radiation Safety Section has updated its form for radiation shielding plan reviews. Form BHS/HFS-852 is now called "Application for a Radiation Shielding Plan Review" and is available on our website. A copy of the form is also attached to this e-mail. We encourage anyone that needs a shielding plan review to immediately begin using this updated form.

Form BHS/HFS-852 is a fillable PDF form which allows the user to type information directly into the form. Data entered into the form can be printed but cannot be saved. We would appreciate if you could report any problems with the form to our office, either at (517) 241-1989 or by e-mail at rssinfo@michigan.gov.

As always, an application for a radiation shielding plan review should be submitted and approval obtained before construction and before operation of the x-ray unit. We do not charge a fee for this service. Radiation shielding guidance can be obtained from our website.
