WHAT IS IT?
The Michigan Dental Program (MDP) covers dental care for persons living with HIV/AIDS who qualify for the program.
WHO QUALIFIES FOR THE PROGRAM?
To receive assistance from the Michigan Dental Program (MDP), applicants must meet the following criteria:

Applicant must provide documentation of HIV disease.

Applicant must be a resident of the State of Michigan.

In some cases, applicant must have applied for public assistance (Medicaid and/or Adult Benefits Waiver program) with the Department of Human Services (DHS) within the past 90 days and have a pending, denial, or spend-down status. Clients being renewed by April 1, 2008, must have applied with DHS after October 1, 2007.

Applicant's gross income cannot exceed 450% of the Federal Poverty Level (F.P.L.) and will be evaluated based on FPL guidelines in effect when MDP receives your completed application.
If you are currently receiving medical and prescription coverage under the Adult Medical Program/Adult Benefits Waiver (AMP/ABW) case, you may apply for assistance from the MDP for your dental care.
If you have Private Dental Insurance (Delta Dental, BC/BS, etc.) you are not eligible for the MDP.
WHAT SERVICES ARE COVERED?
Covered services are based on pre-approved care plans and include both preventive and restorative care.
HOW DO I OBTAIN SERVICES?
Case Management/Client Advocacy agencies can assist applicants in completing the MDP application. Completed applications will be reviewed and accepted or denied. When accepted by the MDP, you will receive an acceptance letter with a personal identification number You will also receive a copy of the letter for your dentist. The dentist will bill the MDP directly, based on pre-approved care plans, for the cost of dental services. The MDP can assist you in locating a provider in your area.
