THE STRATEGIC PLAN FOR THE DIVISION OF HEALTH, WELLNESS AND DISEASE CONTROL

The Strategic Plan:  The Foundation

 

The Vision of the State of Michigan

 

The Division of Health, Wellness & Disease Control is operated within the Department of Community Health, which is part of the State of Michigan.  The State's Leadership Blueprint identifies the following as the Vision for all of its Departments and Divisions:

 

 

"Everything we do in our daily jobs is aimed at providing the highest quality service to our customers."

 

 

The Mission of the Department of Community Health

 

The Department of Community Health strives for a healthier Michigan, guided by its Mission to:

 

Ø      "promote access to the broadest possible range of quality services and supports;

 

Ø      take steps to prevent disease, promote wellness and improve the quality of life

 

Ø      strive for the delivery of those services and supports in a fiscally prudent manner"

 

The Mission of the Division of Health, Wellness & Disease Control

 

The Mission of the Division of Health, Wellness & Disease Control is to promote the public health and provide leadership to:

 

Ø      Prevent the spread of HIV and STDs

 

Ø      Provide care to those infected and affected by these diseases

 

Ø      Reduce health disparities by supporting a portfolio of social/behavioral interventions that will have the greatest impact among racial and ethnic minorities

 

Ø      Utilize science-based strategies with proven effectiveness

 

Ø      Deliver quality prevention and care initiatives with highly skilled and culturally competent staff

 

The Values of the Division of Health, Wellness & Disease Control

 

The Division of Health, Wellness & Disease Control will conduct itself with:

 

Ø      Integrity, to assure that honesty, trustworthiness, and credibility guide the conduct of the Division of Health, Wellness & Disease Control staff;

 

Ø      Commitment to the cause, to assure the Division of Health, Wellness & Disease Control's resources are maximized and focused on the ever-changing issues of minority health, health disparities, HIV and STDs;

 

Ø      Customer focus, to assure that the highest quality services are delivered;

 

Ø      Commitment to quality, to assure excellence in staff performance and the achievement of the best outcomes for its customers;

 

Ø      Cultural Competence, to assure the conduct of the staff and the programs are responsive to the diversity and cultural attributes of the people served;

 

Ø      Integrated Services, to assure the most efficient, effective and streamlined approaches in designing and delivering services by integrating HIV/AIDS and STDs.
