| Title V Maternal and Child Health Block Grant Application, 2009 |

Each year, the Department of Community Health is required to submit to the U.S. Department of Health and Human Services an application for funding from the Title V Maternal and Child Health Block Grant. This funding supports programs targeted to pregnant women, mothers and infants, children and adolescents and children with special health care needs. Public comment is required on the draft application. A hard copy may be obtained by calling 517-335-8946. Comments may be submitted to oganc@michigan.gov or sent in writing to:
