Health Insurance

Private Insurance & Medicaid 

The Health Insurance Verification Unit identifies Medicaid beneficiaries who have private health insurance or any other payment source. 

Just Ask - Coordination of Benefits 

What information does Medicaid need regarding my private health insurance? 

  • Name and phone number (if known) of the Insurance Carrier 
  • Policy Holders name and birthdate (if known) 
  • Policy number 
  • Group number 
  • Start and end dates (if known) 

Most of this information is usually found on your Health Insurance card. 

Who do I contact for access to care issues? 

DHS - 1-800-292-9570 option 5 

Providers/Pharmacies - 1-800-292-2550 option 4 

Form DCH-0078 and any other Inquiries can be faxed to 1-517-346-9817 

Medicaid Beneficiaries 

How do I let Medicaid know when I have private health insurance or changes to my exisiting insurance. 

Contact your local  Dept. of Human Services (DHS) office . 

Beneficiary Helpline 1-800-642-3195 

Insurance information can also be given to your primary care physician at time of service.

Statutes 

Public Act 593  became effective Jan. 1, 2007 and provides for the sharing of health care coverage information with the Department of Community Health.  In addition, claims submitted within three years of the date of service shall not be denied based on the date of submission.
