| FOIA Policy |
As the Department of Community Health is a public body required by law to provide public records to persons requesting public records unless those records are exempt from public disclosure by the Freedom of Information Act (FOIA) or some other statute.
An official Freedom of Information Act (FOIA) policy, effective November 1, 2004, is now available for review by clicking on the link above. The policy, as linked above provides specific detail regarding definitions, general information, exemptions under FOIA, procedures for processing requests, as well as new fees.
Attached to the policy are Exhibit A - Response to Request for Public Records; Exhibit B - FOIA Fee Calculation Record; Exhibit C - Statement of Fees; Exhibit D - FOIA Requests for 2004.
