• Verification of Record Change to a Michigan Death Record
  This application form is to be used to verify whether a change was made to a Michigan death record. If a change is found, you will receive a copy of the application and a letter from the Changes Unit. If a change is not found, you will receive a letter indicating that.
  
  
• Physician Application to Correct Michigan Death Record before 1/1/2004
  This form to be used by the physician if death occurred PRIOR to January 1, 2004.
  
  
• Physician Application to Correct Michigan Death Record AFTER 1/1/2004
  This form to be used by the physician if death occurred AFTER January 1, 2004.
