The Michigan Department of Community Health (MDCH) has summarily suspended the pharmacist license of Howard Allen Applebaum, R.Ph.
The MDCH Bureau of Health Professions orders a summary suspension when it believes that the public’s health, safety, and welfare warrant emergency action. The suspension is a result of an investigation conducted by the federal Drug Enforcement Administration, which revealed Applebaum’s distribution of controlled substances to individuals without a physician’s authorization.
On November 16, 2004, MDCH issued an order immediately suspending Applebaum’s license pursuant to the Public Health Code. An administrative hearing will be scheduled to address the status of Applebaum’s pharmacist license.
