• MDCH has compiled a  HIV Testing Guidance Document  that provides information on HIV informed consent, confidentiality and release of HIV information, and counseling and reporting of HIV positive tests geared towards Health and Life Insurance Companies.
  
• Additionally,  Michigan law  requires that positive HIV test results be reported to the local or state health departments (MDCH) withing 7 days of receiving the test result. 
  
• Positive test results must be reported to the health department on the approved  Adult Case Report Form , if needed,  instructions to the ACRF  are also available.
  
• Please mail the completed case report form, along with any additional lab reports, in an envelope marked "confidential" directly to:
