    Welcome to the Primary Care and Public Health Web Page

    Purpose The purpose of this web site is to support the growing partnership between public health and primary care.  Public health focuses on offering a broad array of services across communities and populations that collectively will help people to be healthy.  Primary care focuses on providing medical services to patients with acute, preventative and/or chronic conditions.  Together we share a goal of promoting the health and well-being of all people.



    Self-Management for Patients

    • Diabetes Self-Management Education and Support: A process which incorporates the needs and goals of a person with diabetes or prediabetes to improve and sustain their health and quality of life.

    • Michigan Partners on the PATH: The Stanford Chronic Disease Self-Management Program in Michigan PDF icon PATH,(Personal Action Toward Health) is an evidence-based program designed to teach people useful techniques and strategies for active day-to-day management of chronic or long-term health conditions.

    • Agency for Healthcare Research and Quality: Free research summaries about the benefits and risks of different treatments for different health conditions for both providers and patients.
