The State Office of Recipient Rights Annual Report 2009-2010 is presented in 2 documents.  The "Annual Recipient Rights Report, Part I" contains all the report requirements mandated in the Michigan Mental Health Code, including Appendix A(the data template) and Appendix B, the individual complaint data for each of the state operated hospitals and centers.  The "Annual Recipient Rights Report, Part II" contains the consolidated complaint data for the Community Mental Health Services Programs (Appendix C), information regarding remedial action taken by CMHSP's (Appendix D), consolidated complaint data for all Licensed Private Hospitals and Units (Appendix E) and information regarding remedial action taken by LPHU's (Appendix F)
