Integrating Mental Health, Substance Use and Physical Health Care

Achieving Integration of Physical and Behavioral Health Care .  Presentation at the Michigan Association of Community Mental Health Boards Spring Conference-May 18, 2010

Bay-Arenac Behavioral Health- Developed a community based heath care coordination model that includes a full time Certified Peer Support Specialist (CPSS), PATH and other wellness classes.

Bureau of Substance Abuse and Addiction Services (BSAAS).   A Mental Health and Substance Abuse Administration effort to promote Treatment of Co-occurring Disorders.

Centra Wellness Network  (aka Manistee-Benzie CMH) - Established clinic in local rural primary care clinic. The clinics share medical and behavioral health staff and CPSS run PATH classes. 5/19/11 Update-Name change to reduce stigma and increase access at primary care sites and currently has signed agreements with our area FQHC to provide Behavioral Health Services in their clinics.

CMH for Central Michigan - Employed a full time CPSS who leads a peer-led initiative that assures consumers are receiving appropriate care. Also hold PATH and other wellness classes.

Community Mental Health Authority of Clinton, Eaton and Ingham Counties - Primary and Behavioral Healthcare Integration:  Status of Efforts to Date - January 2011

Detroit-Wayne County CMH Agency - Partnered with local FQHC to have co-located physician and nurse. Peers are also PATH certified to co-facilitate groups.

Genesee County CMH Services- 2 tiered project

  • Tier I - Implemented evidence based InShape program.
  • Tier II - Have CPSS providing support in the ERs of 2 different hospitals.

Lapeer County CMH Services - Creating a data warehouse to allow sharing of critical health-care related data and encounters between CMHSP and Health Plan.

Muskegon County CMH Services - Partnered with local FQHCs to embed 2 outpatient CMH staff in the FQHC and have a nurse work at the CMH.

Network 180 - Partnering with local FQHC to build a new clinic that is designed to house both behavioral health and primary health services on the same floor.

Oakland County - Created data sharing and high utilizer projects to identify the population of consumers, who in spite of utilizing a high number of services are not achieving the positive outcomes they desire.

Project Launch-Implementation in Saginaw County .  Integration of Early Childhood Mental Health Consultation in Primary Care Settings

Southwest Counseling Solutions and Covenant FQHC - Southwest Counseling Solutions and Covenant Federally Qualified Health Center partner to provide on site integrated health care in the same building.

Summit Pointe - Provides INTEGRATED HEALTH CARE services through its wholly-owned primary care practice that is housed within the CMHSP. CPSS provide a range of recovery based services including PATH classes


  • Summit Point Integrated Block Grant Briefings: "A series of briefing papers based on the research and experience of Summit Pointe in creating and managing an embedded Primary Care Practice within a Community Mental Health Center."


Washtenaw Community Health Organization - Created a Pilot Disease Management Project (PDMP) that identifies individuals with SPMI who have or are at risk of developing chronic health conditions and makes sure they have access needed primary care treatment and other health and wellness activities.
