ELIGIBILITY
Employees
As
an employee, you are eligible for State Vision Plan coverage if you are
in any category of Classified State service with an appointment of at
least 720 hours duration and if you are:
- A full-time (FT)
employee
- A part-time (PT)
or job sharing (JS) employee working 32 hours or more every bi-weekly
pay period
- A permanent intermittent
(PI) employee expected to work every bi-weekly pay period and at least
40 percent of full-time annually (a minimum of 832 hours)
Your eligibility
date is the first day of the bi-weekly payroll period following your completion
of one full payroll period of employment.
Dependents
You may cover your
wife or husband as a dependent as well as eligible dependent children
up to age 19. These dependent children can include:
- Your children by
birth, legal adoption, or legal guardianship while they are in your
custody and dependent on you
- Any other unmarried
child who lives with you in a parent-child relationship, regularly attends
school, and depends solely on you for support
- Your children by
birth or legal adoption who do not reside with you, but are your legal
responsibility for the provision of medical care (for instance, children
of divorced parents)
When
properly enrolled, your dependent’s State Vision Plan coverage is
effective the same day as yours unless your dependent is confined in a
hospital and the confinement began prior to the effective date of your
enrollment. The dependent’s coverage will be delayed until he or
she is discharged from the hospital.
Your
dependents as of your eligibility date are also eligible for State Vision
Plan coverage on the same date. Dependents acquired after your enrollment
are eligible on the date they are newly acquired (by marriage, divorce,
birth, adoption, residency in your home, etc.).
Existing
coverage is automatically continued for incapacitated children age 19
and over. See the Incapacitated Child Continuation provisions under the
“Termination of Dependent Coverages” section.
No
person will be considered a dependent while in the armed forces of any
country.
Dependent
Children Between the Ages of 19 and 25
Dependent
children who meet the eligibility requirements listed below may continue
to be covered under the State Vision Plan as a continuation member after
they reach age 19. This dependent coverage may continue to age 25, if
they remain eligible. Coverage for these dependents will be exactly the
same as yours.
To
be eligible, these dependents must meet all of the following requirements:
- Be unmarried and
between the ages of 19 and 25
- Be solely dependent
on you for their support
- Be living with
you in a parent-child relationship
- Be a student who
regularly attends school
No
person may be covered both as an “employee” and “dependent”
or as a dependent of more than one employee. If the State of Michigan
employs both you and your spouse, vision coverages may be carried separately
or as one family enrollment with dependent coverage.
In
the event the husband or wife who is enrolled as the “employee”
separates from State service or enters a leave of absence or layoff, coverage
may be transferred at any time to the remaining State employee provided
that the remaining employee:
- Meets the eligibility
requirements for enrollment
- Was covered as
a “dependent” of the departing employee or was enrolled
separately as an “employee”
- Notifies his or
her human resources office of the desire to transfer enrollment prior
to the departure of the spouse from State service
