Cancellation
You
may cancel your employee Vision Plan enrollment at any time. The cancellation
effective date is the last day of the last payroll period in which a premium
contribution is paid.
You
may cancel your dependent’s coverage at any time. The cancellation
effective date is the last day of the last payroll period in which a dependent
contribution, if required, is paid.
In
the event of divorce, the cancellation effective date is the date of divorce.
Termination of
Employee Coverages
Your
coverage under the State Vision Plan will automatically terminate (except
as explained in the following NOTE) in the event of the following, whichever
occurs first:
- When your employment
terminates
- When you are no
longer in an eligible classification of employees
- On the last day
of the last payroll period for which you made any required premium contribution
- When the entire
group contract is discontinued
NOTE: Continuation
of Terminated Employee Coverages
You
may be eligible to continue State Vision Plan coverages for a period of
time under the Continued Group Insurance System (CGIS). This can be done
by making direct, full premium payments (including the share of the premium
that was paid by the State) in the event of a separation from employment
(except retirement), a reduction of hours, a leave of absence or layoff.
Contact your human resources office for premium rates and a CGIS application
form (DMB-1499 OSE).
The
continuation opportunity will end if you fail to make application or if
you fail to pay the full CGIS premium within stated time limits.
Termination of
Dependent Coverages
Your
dependent’s coverage will automatically terminate (except as explained
under the “Death” and “Eligibility” sections and
in the following NOTE) in the event of the following, whichever occurs
first:
When your dependent
becomes eligible for coverage as an employee
When your coverage
terminates
When your dependent
no longer meets the definition of an eligible dependent
On the last day of
the last payroll period for which you made any dependent premium contribution
that may be required
When the entire group
and/or the group dependent contract are discontinued
NOTE: Continuation
of Terminated Dependent Coverages
Your
dependents may be eligible to continue terminated State Vision Plan coverage
for a period of time under the Continued Group Insurance System (CGIS).
This can be done by making direct, full premium payments (including the
share of the premium which was paid by the State) in the event coverage
is lost because of your:
- separation from
State employment (except retirement)
- leave of absence
or layoff, or because of your death
- divorce
- dependent’s
age ineligibility
Contact
your human resources office for premium rates and a CGIS application form
(DMB-1499 OSE).
This
continuation opportunity will end if your dependent fails to make application
or fails to pay the full CGIS premium within stated time limits.
Continuation
of Coverage for Incapacitated Children
If
your dependent is an incapacitated child, your coverage for the child
will automatically continue at and beyond age 19 as long as he or she
continues to be incapacitated and provided coverage does not terminate
for any other reason. Your child will be considered incapacitated if he
or she is unable to earn his or her own living because of a mental or
physical impairment and he or she depends chiefly on you for support and
maintenance. Proof that your child is incapacitated must be submitted
periodically. Contact your human resources office for additional information
if your enrolled child is incapacitated.
