If you add new dependents to your insurance coverage during open enrollment, you must mail or fax the appropriate proof of eligibility documentation* to the MI HR Service Center by September 30, 2011 for the enrollment to be valid. Click the link below to view a listing of valid documents.

Required Documentation for Dependents and Dependent Eligibility Guidelines 


State Police Employees Only: You must mail or fax the Evidence of Insurability form and Life Insurance Enrollment form to the MI HR Service Center no later than August 31, 2011.
