Pay & Deductions Codes	Search pay warrant and deduction code defintitions.	View information.
Direct Deposit	View, add, update, or revoke up to four direct deposits.	You are responsible for the accuracy of any changes.

Credit Union Deductions	Directions for transferring Credit Union Deductions to Direct Deposits.	User Guide NOTE: Credit Union Deductions pre-date the Internet and cannot be managed online.

Menu Item	Purpose	Notes
Personal Profile	Lists your personal work-related information like name, nickname, SSN, birth date, gender, EEO class, marital status, veteran status, former name and maiden name.	View your current records. Report any discrepancies found (see Who do I contact if my records are incorrect? ).
Leave Balances	View your current leave balances, continuous service hours and maximum accrual hours.	View your current records. Report any discrepancies found (see Who do I contact if my records are incorrect? ).
Emergency	Enter and Update your emergency contact information.	Keep your emergency contact information up to date.
Home Address	View your current address on file.	Contains link for address change. Update your address and/or phone number when changes occur. If you move into or out of a city that collects income tax, you MUST complete the Tax Withholding forms (see Pay >> Tax Withholding)!
Applicant Pools	View your status within the applicant pools maintained by Civil Service.	View your current records.
Training History	Lists Civil Service Training History as of 10/01/02.	View your current records. NOTE: Only Civil Service offered training is listed.
Certifications	View the status of your work related certifications & licenses.	View your current records. NOTE: Additions and changes can only be made by your HR Office.

Current Benefits	View your current benefit coverage, pre-tax and after-tax cost and the employer share of bi-weekly costs.	View your current records.
Dependent Benefits	View your dependent benefits with start and stop dates.	View your current records.
Beneficiary	Update beneficiary designations on-line for Employee Life Insurance and Last Check (Final Compensation). Print designation forms for other benefits.	View your current records. Keep beneficiary designations up-to-date; submit completed forms as indicated.
Express Scripts	Link to Express Scripts (third-party provider for State Health Plan prescriptions).	View web site.
ADP-FlexDirect	Link to third party provider for flexible spending accounts.	View web site.
401K/457 Plans	Link to CitiStreet website for state employees; also links to 401k and 457 plan-related forms.	View web site.

Menu Item	Purpose	Notes
Divorce	Update your records to indicate a divorce. Prompts you to consider related changes to address, dependent(s) address, emergency contacts, tax withholding, direct deposit, beneficiaries, and benefits/insurance.	Enter divorce and name change information; submit copy of divorce papers as directed. If you want to change your name, also submit social security card copy with the new name. To drop insurance coverage, you MUST contact the MI HR Service Center or your HR Office!
Marriage	Update your records to indicate a marriage. Prompts you to consider related changes to address, add a dependent, dependent address, direct deposit, tax withholding, emergency contacts, beneficiaries, and benefits/insurance.	Add spouse to your records when married; submit copy of marriage license as directed. If you want to change your name, also submit social security card copy with the new name. If you want dependent insurance coverage, you MUST contact the MI HR Service Center or your HR Office!
Move	Update your address and home phone number. Prompts you that other changes may be needed to your: dependent(s) address, tax withholding, direct deposit, emergency contacts, and benefits/insurance.	Update your address and/or phone number when changes occur. If you move into or out of a city that collects income tax, you MUST complete the Tax Withholding forms (see Pay >> Tax Withholding)!
