Talk to us if you have complaints or concerns.

If you have any issues with your child's dentist or dental office, we recommend talking to the dentist who provided the service. If you aren't satisfied, you can request a formal review through the Quality of Care Complaint Procedure. To do this, send us a copy of your Delta Dental statement. Include a letter telling us about your problem, as well as any other facts that might help us.

Be sure to include:

  • Your name
  • Your address
  • Your daytime phone number
  • The date
  • The child's name
  • The child's Social Security or Delta Dental ID number
  • The child's address
