This web site exists to support the growing partnership between public health and primary care. Public health focuses on offering a broad array of services across communities and populations that, collectively, help people to be healthy. Primary care focuses on providing individuals with acute, preventive and/or chronic care for conditions and diseases. Together, public health and primary care share a goal of promoting the health and well-being of all people.
A specific aim of the information found here is to assist Michigan Primary Care Transformation (MiPCT) Project teams in identifying and obtaining information and resources that help them address patient needs and reach MiPCT indicator targets. This web site presents information about:
- Michigan Department of Health and Human Services (MDHHS) programs of interest to primary care practices;
- patient self-management educational resources specific to certain chronic diseases and conditions; and
- other programs and resources of use to healthcare providers and other professionals working in primary care practices.
