Freedom of Information Act Request Form

The Michigan Freedom of Information Act (FOIA) allows the public to submit requests for copies of non-exempt information to the Michigan Department of Transportation by fax and E-mail. Requests can be faxed to 248-569-3302.

You may be charged for the response to your FOIA request. The cost will be calculated at $0.17 per page plus $19.80 per hour labor charge. Postage and other charges related to the shipment will be on an actual cost basis.




All requests must describe the desired documents adequately and in sufficient detail for staff to identify and locate the information.
