Procedure To Request A Copy Of A Traffic Crash Report Investigated By The MSP*

On the RI-101 (10/99) Request for Public Records (Michigan Freedom of Information Act) you must complete a minimum of the first four items below:

  • First section of the form called "To Be Completed by the Requestor".

  • Type of report (incident number)

  • Date of the event (date of accident)

  • Location of the event (accidents should include the street location, county, city/township)

  • Complete any other information you are able to in an effort to locate the report more easily.

* Copies of Traffic Crash Reports must be requested from the investigating agency ( i.e. sheriff, local law enforcement).
