LEOKA Reporting
Starting in 2008, all Law Enforcement Agencies using AICS will be required to complete the following LEOKA form to submit their law enforcement officers killed or assaulted information to the state.

The LEOKA form is to be completed and submitted to the AICS/RMS unit via e-mail at AICSMAIL@michigan.gov or fax to 517-241-1644 prior to the 10th of every month following the reporting period.

For example: any assaults and/or deaths that occur to an officer in January of 2008, must be reported by the 10th of February. February assaults and/or deaths, must be submitted by the 10th of March, etc.
