| How do I file a complaint? |

Fill out and submit the OCO complaint form online.
Call the OCO at (800) 642-4326 (MICH-FAM) or (517) 373-3077 to request a complaint form and a postage-paid envelope, and mail your complaint.
Call the OCO at (800) 642-4326 (MICH-FAM) or (517) 373-3077 during business hours, Monday to Friday, 8 a.m. to 5 p.m. (or leave a message after hours or on weekends) with the following information:
- Your name, address, and phone number
- Child or children's names and dates of birth
- County DHS office or private agency involved
- Brief description of the problem
- Action(s) you are requesting from OCO
An OCO intake investigator will contact you to gather additional information. If the OCO determines that your complaint will not be investigated, you will be provided with information to assist you in resolving your complaint.
All contact with the OCO is strictly confidential. We do not disclose the identify of people who file complaints with the OCO unless they give us their permission. OCO records are confidential and are exempt from disclosure under the Freedom of Information Act (FOIA) and are not subject to court subpoena.
